    Description

    he ICMS Case Manager will provide trauma-informed, client-centered, and housing-first case management services to individuals and families enrolled in our supportive housing programs. This position requires a reliable vehicle, current auto insurance, and a valid California driver’s license, as travel throughout Los Angeles County is a key component of this role. This is a temp-to-hire position, and we are looking for a candidate interested in committing to this mission-driven work for at least 2–3 years.

    Key Responsibilities:

    + Provide Intensive Case Management Services (ICMS) to a caseload of 15–20 clients, focusing on housing stability, mental and physical health, and community integration.

    + Conduct home visits and field-based case management in compliance with program and funder requirements.

    + Develop individualized service plans in collaboration with each client, ensuring client engagement and progress toward personal and housing goals.

    + Coordinate with external providers including mental health professionals, substance abuse counselors, healthcare providers, and employment specialists.

    + Assist clients in obtaining and maintaining public benefits, employment, and other supportive services.

    + Maintain accurate and up-to-date case notes, assessments, and documentation in HMIS and internal databases.

    + Participate in team meetings, case conferencing, and ongoing training to ensure best practices in service delivery.

    + Respond to crises and emergencies in a timely and professional manner.

    + Build and maintain strong relationships with landlords and housing partners to support housing retention and prevent eviction.

    Requirements

    + Bachelor’s degree in Social Work, Psychology, Human Services, or related field (Master’s degree preferred).

    + 1–2 years of experience providing case management or direct services to homeless populations, particularly with ICMS or PSH programs.
