    Job Title: Licensed Practical Nurse Care CoordinatorJob Description

     

    We are seeking a dedicated Licensed Practical Nurse Care Coordinator to join our team. The role involves preparing patients for visits, maintaining exam rooms, and ensuring accurate documentation in electronic medical records. You will play a vital role in patient care and coordination by handling messages, prescriptions, and follow-ups, while adhering to infection control and patient confidentiality protocols.

    Responsibilities

    + Prepare patients for visits by assessing chief complaints, reconciling medication lists and allergies, and evaluating smoking status.

    + Prepare and maintain exam rooms by restocking clinical areas, sterilizing instruments, and maintaining medical equipment.

    + Document interventions accurately in the electronic medical record and review charts for completeness, test results, and billing.

    + Scan upcoming appointments for additional medical record information and obtain necessary records from various healthcare facilities.

    + Respond to patient messages, manage prescription refills, lab results, referrals, home health agency communications, and medication prior authorizations.

    + Collect patient satisfaction surveys and contact patients between visits to assess medical status and medication adherence.

    + Follow protocols related to infection control, environmental safety, and patient confidentiality, completing annual recertification for OSHA, Compliance, Respiratory FIT, and IT policy.

    Essential Skills

    + Practical Nurse License

    + Experience in ambulatory care and diabetes/endocrinology preferred

    + BLS certification

    + Knowledge of medical records and health care systems

    + Proficiency in Epic and electronic health records

    + Managed care experience

    Additional Skills & Qualifications

    + Experience in endocrinology preferred but not required

    + Licensed Registered Nurse qualification

    + Triage experience

    Work Environment

    Our outpatient office offers a learning environment where employees have opportunities for permanent roles and skill development. The family medicine department provides exposure to a variety of illnesses and treatments. Work hours are Monday through Friday, 8 AM to 5 PM, with a one-hour lunch break. There are no weekend or holiday shifts, with minimal overtime expected.

    Pay and Benefits

    The pay range for this position is $27.00 - $30.00/hr.
