    OverviewEnsures the overall delivery of home health services in accordance with VNS Heath policies, procedures and standards of practice. As part of the healthcare team, supports the VNS Heath mission, vision, values to ensure the highest level of patient-centered care, provides clinical oversight, collaboration with the healthcare team, performs administrative duties to optimize patient care and resources and supervises scheduler and scheduling functions. Works under general supervision of Branch Director.

     

    Compensation Range:$62,400.00 - $72,000.00 Annual

     

    • Responds to customer, patient, and caregiver inquiries regarding scheduled visits as back-up and needed, care delivery, service concerns, and updates • Completes inquiry and intake forms for all service requests. • Works with scheduler, patients, families, and healthcare team to coordinate home health visits, ensuring care is delivered timely and as prescribed. • Monitors patient care schedules to ensure the timely delivery of skilled services, supplies, and other care needs, maintaining continuity of care by adjusting schedules as needed and resources based on patient needs and provider availability. Ensures compliance with care plan protocols, regulatory requirements, and patient safety standards • Identifies new approaches to care delivery and customer services to promote consumer experiences. • Provides guidance and clinical oversight to ensure that care plans and services align with patient needs and physician orders. • Serves as a liaison between field staff (nurses, therapists) and the clinical team, offering support as necessary. • Addresses any clinical concerns, escalating them as needed to the appropriate Clinical Field Manager or Branch Director. • Collaborates with scheduler, patients, caregivers, clinical staff, and physicians to ensure all necessary documentation and authorizations are in place prior to scheduling a visit. • Communicates any changes or delays in the schedule to patients, families, and providers. • Maintains up-to-date communication and patient records in the scheduling system, ensuring accurate documentation of visits, changes, and cancellations. • Coordinates staff availability, ensuring optimal staffing levels based on patient needs and provider schedules. • Tracks and ensure compliance with Medicare, Medicaid, and insurance guidelines and regulations regarding home health services. • Ensures timely discharge and missed visit to reduce re-hospitalization. • Ensures that home health care teams are compliant with any company-specific policies and procedures. (such as tracking of NOMNC and ABN) • Supervises scheduler within the team and tracks and reviews HHA supervisor weekly visit. • Monitors patient care schedules as needed to ensure compliance with care plan protocols, regulations, and patient safety standards. • Assists with audits, data entry, and documentation for quality assurance purposes. • Adjusts schedules as necessary due to emergencies, last-minute cancellations, or patient change in condition, ensuring minimal disruption to patient care. • Manages team PTO schedules, reviews payroll, and escalates to CFM/BD for approval; identifies individual performance issues and escalates to management as needed. • Administrative support as required, such as maintaining patient charts and scheduling follow-ups • Runs productivity report for Clinical Field Manager and Branch Director to manage staff’s productivity. • Identifies issues regarding individual team member performance and escalates to management. • Provides care coordination through a collaborative process of planning, facilitation and advocacy for options and services to meet patient’s health needs along with the multi-disciplinary team. Communicates resources and services available to patients through the continuum of care. • Collaborates with the interdisciplinary clinical team, under the guidance of an RN, to review clinical reports of providers for relevant patient data. Communicates patient condition to Clinical Field Manager and case manager in a manner that is focused and reflects knowledge/understanding of patient condition/progress. • Facilitates communication and coordination among patients’ health care team, involving the patient in the decision-making process in order to minimize fragmentation of services. • Participates in special projects and performs other duties as assigned.

    QualificationsLicenses and Certifications:

    LIC - Licensed Practical Nurse - New York State required

    Education:

    High School Diploma or completion of LPN program required

    Work Experience:

    Minimum of 2 years of experience in a home health or clinical setting, including experience with patient scheduling and coordination required Experience in managing patient care plans, monitoring progress, and adjusting as needed required Excellent communication and interpersonal skills, with the ability to interact with patients, families, and healthcare professionals required Strong organizational skills with the ability to manage multiple schedules, prioritize tasks, and solve problems efficiently required Proficiency in base computer software, electronic health records (EHR), and scheduling platforms required Basic clinical knowledge to assess and ensure appropriate care delivery required
