Job Description:
The Licensed Vocational Nurse Coordinator works under the supervision of a Registered Nurse. The Licensed Vocational Nurse Coordinator will assist the Hospice team to assure quality patient care. The Licensed Vocational Nurse Coordinator respects and promotes the mission, values and vision of VNA Health.
Job Duties:
+ Knowledge of state, federal, local and accreditation regulations for the delivery of hospice services.
+ Performs admission and discharge audits to insure all needed documents present and signed appropriately.
+ Assist with compliance by gathering information for Hospice Managers review in regards to visit frequency, orders. RN supervision, certifications and recertifications.
+ Run unverified visit report daily and report findings to Hospice Managers.
+ Resource to field staff to assist as needed with calling medications to pharmacies, faxing documents to facilities or Doctors and follow up calls to Doctors.
+ Answer patient phone calls and relay messages and information to Hospice team.
+ Assist with scheduling of routine, admission, continuous care and on call visits.
+ Home or facility visits as needed.
+ Assist with Notice of Election of Hospice as needed.
+ Assist with new employee orientation.
+ Assist with new employee Netsmart training.
+ Become a super user of Netsmart for staff support.
+ If making patient visits provide direct patient care as defined in State Nurse Practice Act.
+ If making patient visits implements current nursing practice following a patient assessment and an approved plan of care initiated by the Registered Nurse.
+ If making patient visits provide accurate and timely documentation of patient services to reflect the plan of care.
+ If making patient visits assess and provide patient and family/caregiver education and information pertinent to diagnosis and self plan of care.
+ If making patient visits participates in coordination of hospice services, appropriately reporting the identified needs for hospice interdisciplinary group members. Examples include hospice aide, OT, PT, MSW, ST, Dietitian or Hospice Manager.
+ If making patient visits use equipment and supplies effectively and efficiently.
+ If making patient visits provides appropriate pain/symptom management. Evaluates patient’s response to treatments/medications.
+ Participates in personal, professional growth and development. Also participates in organization's quality management program.
+ Performs other duties as assigned by the Hospice Director.
+ Actively participates in Quality management program.

All clinical staff are expected to participate as needed in variable shifts, including weekend rotation per staffing needs and departmental holiday rotation.
Qualifications:
+ Current and active California Licensed Vocational Nurse (LVN) license.
+ Minimum one year experience required, three years’ experience preferred. Community health/hospice or medical/surgical experience is preferred.
+ Current CPR certification.
+ Licensed driver with insured automobile - in accordance with state and/or organization requirements.
+ Complies with accepted professional standards and practice.
+ Demonstrates good verbal and written communication, and organization skills.
+ Understands philosophy of hospice concept. Also understands needs of the terminally ill.
+ Bilingual – Spanish preferred, but not necessary.
Physical Requirements:
+ Ability to lift 50 lbs.
+ Ability to push/pull 50 lbs.
+ Ability to stoop, bend, and squat.
+ Ability to walk up one flight of stairs.
+ Ability to walk one mile.
+ Ability to sit for one hour, minimum.
