RN Home Care Manager -** **Full Time - Eddy SeniorCare
Eddy SeniorCare has a rewarding opportunity for an RN Home Care Manager to work in our growing Program of All-Inclusive Care for the Elderly!
PACE is a unique program on the forefront of trends in healthcare, combining an insurance role (we are the Medicare/Medicaid insurer for our participants) with a comprehensive healthcare provider role (primary care, home health, adult daycare, transportation, all coordinated by an interdisciplinary team).
The primary purpose of your job position is to assess participant homecare needs, develop and update home health aide care plans, initiate aide services & complete home health aide supervisions. Such supervision must be in accordance with current state, federal and local standards, guidelines, and regulations that govern our program, and as may be required by the Director of Participant Services, to ensure that the highest degree of quality care is maintained at all times.
The RN Home Care Manager will work with the RN Case Management team and entire interdisciplinary team to ensure requests for home care services are processed in a timely manner, and communicate any barriers that prevent services from be provided.
The position will also include the need for PRI/UAS certification. Training to be provided if necessary
Work Schedule and Location:
Monday - Friday 8:00am-4:30pm** **This position will be based out of our Rotterdam location but will also provide support for the homecare program at our Latham site and may involve travel to that location at times.
No Holidays, No Weekends, No On Call!
Requirements:
+ NYS RN licensure.
+ Two or more years experience.
+ Experience and desire to care for seniors.

Pay Range: $36.50 - $49.95
Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
