    Description:

    Position will be located at Cabrini Clinic, 1234 Porter Street, Detroit, MI 48226.

     

    The Community Health Worker (CHW) is engaged in the community served, connecting patients within our Detroit Clinics and partnering organizations. The position conducts outreach to help individuals connect to social service organizations as part of a team focused on improving health outcomes for patients throughout metro-Detroit. The CHW serves as a liaison/link/ intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of clients. In addition, the CHW builds individuals and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach community education, informal counseling social support and advocacy.

    REQUIRED EDUCATION, EXPERIENCE AND** **CERTIFICATION/LICENSURE

    High school diploma or general education degree (GED) required. One to three years related experience. Valid Driver's license required. Michigan Community Health Worker Alliance certification preferred.

    ESSENTIAL FUNCTIONS AND RESPONSIBILITIES

    + Improves access to healthcare services through in-person interactions, screenings, telephone contacts, coordination of referrals and services by cross-marking services. Conducts health screening and social assessments.

    + Provides health and wellness education targeting. Promotes understanding of self-management by the client.

    + Understands and can assess and support trauma informed care for students, parents and staff.

    + Assists in facilitating enrollment into federal, state, and local programs.

    + Provides in-service education to service providers relative to healthcare customs/beliefs of the target population and works cooperatively with other community agencies to promote culturally appropriate services.

    + Maintains frequent contact with community agencies serving vulnerable population and participates in school meetings, health fairs and community meetings.

    + Adheres to all ongoing training and/or updates for application eligibility for healthcare coverage.

    + Maintains a working knowledge of applicable Federal, State and local laws and regulations.

    REQUIRED SKILLS AND ABILITIES

    + Must possess a comprehensive knowledge of the local community.

    + Have the ability to write simple correspondence, effectively interview clients and accurately record medical information.

    + Ability to effectively present information in one-on-one and small group situation to clients and other employees of the organization.

    + Ability to effectively relate pertinent information with medical and social service professionals.

    + Ability to drive a vehicle or have access to reliable transportation. Must be able to travel to as needed.

    + Must be able to set and organize own work priorities and adapt to them as they change frequently.

    + Must be able to work concurrently on a variety of tasks/projects in an environment that may be stressful with individuals having diverse personalities and work styles.
