We are currently seeking a **RN Case Manager** that is looking for an exciting new opportunity to join a top 10 nationally ranked provider of end-of-life care. The office for this role is out of the Greenville, SC area, but the patient area is in the Spartanburg, SC area. This is a **full-time, permanent opportunity** with excellent benefits and a salary range based on experience. Position Summary:
+ Responsible for providing nursing services and education within the scope of practice as set forth by the South Carolina Board of Nursing to individuals and their families in homes or long term care facilities.
+ Coordinates plan of care by working with interdisciplinary team to anticipate and meet patient needs.
+ Serves as the RN member of interdisciplinary team to meet specific plan of care goals.
+ Coordinates closely with related disciplines to ensure professional service delivery.
+ May be asked to provide oversight to NAs, LPNs, volunteers, etc.
Position Highlights:
+ $80,000 - $85,000 annual salary
+ Excellent mileage reimbursement rate
+ M-F position, minimal on-call responsibility
+ Generous PTO policy, retirement, and full benefits offered
Qualifications:
A Diploma or Associates Degree in nursing is required; Bachelors Degree preferred.
+ A current, active, and unrestricted compact (multistate) license as a Registered Nurse (RN) is required.
+ Driver's license required.
+ Current CPR required; CHPN preferred.
+ Minimum 1 year related experience.
+ Experience in direct care of patients with life limiting illnesses required.
+ Computer proficiency is required.
**Pay Details:** $80,000.00 to $85,000.00 per year
