Overview

Our Hospice Licensed Practical Nurses (LPN) are the heart of our organization! In this role, you will support your patients by providing direct care and coordinating with the interdisciplinary team to ensure that both patients and their families have an optimal care experience. This position is for an LPN Monday through Thursday. 5pm-8am. 10 hours clocked in and 5 hours on call to cover the 15-hour shift. This nurse would provide support visits dispatched through triage for patient needs in the Gainesville Branch area. This nurse would be the only nurse in the field during these hours to provide in person visits for Home Team patients.

Responsibilities

+ Provide direct patient care to hospice patients, including appropriate pain/symptom management

+ Implement nursing practice following a comprehensive assessment and plan of care

+ Provide accurate and timely documentation of patient services

+ Assess and provide patient and family/caregiver education

+ Participate in coordination of hospice services

+ Provide appropriate pain/symptom management and evaluate patient response to treatments/medications

+ Participate in professional growth and development opportunities and quality assessment activities

+ Ensure compliance with all state and federal legal and regulatory requirements

+ Additional responsibilities as assigned

Qualifications

+ Graduate of an accredited practical or vocational nursing program with current licensure to practice in state of operation

+ Three years nursing experience. Community health/hospice or medical/surgical experience is

preferred

+ Knowledge of the hospice philosophy of care

+ Commitment to clinical and documentation excellence

+ Must be a licensed driver with an automobile that is insured in accordance with state and/or

organization requirements and is in good working order.

+ Possess and maintain current CPR certification.
