    ONCOLOGY-EMBEDDED CLINIC

    Clinical Pharmacist- Mooresville, NC location

    Novant Health is seeking a Full-Time Clinical Pharmacist, to join our outpatient oncology pharmacy team, to deliver optimal medication management across all dimensions of care, every time.

     

    In this clinical role, the pharmacist is embedded inside a medical oncology clinic to partner with provider and nursing teams to support direct patient care in a fast-paced environment. Pharmacists are valued as an integral part of the clinical oncology team. In this role, the pharmacist will also be able to participate in groundbreaking new clinical trials in partnership with our investigational drug services team.

    Essential functions include:

    + Clinical review, entry and modification of oncology treatment plans.

    + Oncology order verification, hazardous and non-hazardous sterile admixture check.

    + Partnership with clinic providers and nursing to provide remarkable patient care.

    + Proficiency with equipment and software.

    + Participation in committees such as P&T, tumor boards and shared governance.

    + Order set review and development.

    + Medication therapy management and patient counseling.

    + Completes all NH regulatory self-audits, reviews results with one-up leader, and corrects identified opportunities.

    + Develops, designs, and implements service line outcomes projects and clinical initiatives as directed by leadership team.

    + PIC maintains name and signature on applicable Board of Pharmacy (BOP) permit. Facilitates BOP on-site inspections. Signs DHHS application and serves as primary point of contact for the DHHS permit (if applicable). Serves as the primary point of contact for the DEA Permit.

    + Maintains power of attorney for controlled substances.

    Qualifications

    + Education: 4 Year/Bachelor's Degree in Pharmacy, required. Doctorate of Pharmacy, preferred.

    + Experience: Postgraduate year 1 residency or fellowship OR two years of experience as a registered pharmacist in health system pharmacy, required. Oncology experience highly preferred.

    + Licensure/Certification: Current Pharmacy licensure in appropriate state(s) or must be completed within 30 days of start date by either examination or reciprocity, required. Board of Pharmacy Specialties Certification or other applicable certification must be obtained and continuously maintained within 2 years of start date as applicable to the role; excludes PRN team members who may not be eligible to take certification exam, required.

    + Additional Skills (required): Other duties as outlined in Novant Health and facility specific policies and procedures. Ability to drive/travel to multiple facilities/locations as needed. Ability to work as part of a high performing, change ready and resilient team.
