Position Profile
The hospice chaplain provides spiritual care and comfort to terminally ill patients, as well as their families and loved ones in an interfaith approach. Care is in the form of spiritual counseling, prayer, devotional ritual, and grief and loss support. The hospice chaplain is part of an interdisciplinary team that supports people at critical junctures in their lives, always making a difference. Care delivery primarily takes place in patients' home or in a facility where the patient receives care.
Responsibilities
• Develops and maintains therapeutic relationships.
• Conducts comprehensive spiritual assessment.
• Provides therapeutic interventions.
• Provides effective patient/family teaching.
• Supports the death/dying process.
• Collaborates in providing patient care.
• Creates timely and accurate documentation.
• Assures patient safety.
• Partners with community chaplains and spiritual counselors.
• Utilizes resources effectively and efficiently.
• Adheres to HOV standards and facilitates continuously improved processes/services.
• Maintains and enhances professional skills.
• Adheres to high standards of personal and professional conduct.
Minimum Qualifications
• Bachelor's degree in Counseling, Divinity, Theology, Ministry or Pastoral Care from an accredited school.
• Minimum of two years experience providing spiritual support and counseling to clients and family members in a health care setting.
• Demonstration of expertise in spiritual assessment and counseling, and in concepts of death and dying issues.
• Knowledge of concepts and issues for dying for adults and children. Current Arizona drivers license and automobile insurance.
Preferred Qualifications
• Masters degree in divinity, theology or related field.
• Previous hospice experience.
• Bilingual abilities, a plus.
