    Fort Loudoun Medical Center is equipped with a team of more than 200 doctors in more than 29 specialty areas. Our 87,000-square-foot hospital features advanced technology, including state-of-the-art Magnetic Resonance Imaging (MRI), Computerized Tomography (CT), Ultrasound, Diagnostic X-Rays and Women’s Imaging Services, as well as Nuclear Medicine technology unique to our surrounding counties.     Position Summary:

    Provide prescribed pharmaceuticals, medications, information and clinical monitoring for adequate patient care according to professional standards and practices .

    Responsibilities

    + Maintains competency in aspects of drug therapy for all patients for which they provide care, including factors related to patients of all groups.

    + Provides pharmaceutical care by collaborating with physicians, nurses, and other members of the healthcare team in:

    + Reviewing patient medication history

    + Educating patients on drug therapy

    + Developing drug monitoring plans

    + Monitoring patient outcomes

    + Documenting medication related issues in patient chart

    + Interpreting laboratory data

    + Reviewing medication profiles

    + Monitoring pharmacokinetics of drug therapy

    + Monitoring potential adverse drug-drug and drug-food interactions

    + Monitors drug therapy to evaluate appropriateness of use, dose, dosage form, regimen, route, therapeutic duplication and drug interaction.

    + Concurrently monitors use of drugs for compliance with prescribing criteria, guidelines and protocols.

    + Detects, monitors, documents, and reports adverse drug reactions and medication errors.

    + Promotes the use of formulary by converting non-formulary orders to formulary when possible. Coordinates procurement of non-formulary drugs when necessary.

    + Performs medication histories, medication reconciliation and discharge counseling as needed and required.

    + Maintains and updates a patient profile with allergies, height and weight, and current medications.

    + Discusses medication order clarifications with the prescribers, document any changes in patient records, and inform others of medication order changes.

    + Review medication orders for appropriateness and check all doses of IV admixtures and unit dose medications prepared by technicians.

    + Identifies and acts upon cost avoidance in drug therapy by promoting more cost effective regimens, drugs, and by reducing waste.

    + Documents clinical interventions as part of our department’s documentation program.

    + Insures proper technique and accurate preparation of all pharmaceutical products, including oral, IV admixtures, chemotherapeutic, and investigational agents.

    + Ensures the timely and accurate dispensing of drugs and solutions from the pharmacy area.

    + Responsible for the accurate filling and dispensing of drugs and solutions from the pharmacy area.

    + Review physician orders for possible therapeutic problems, contraindications, interactions, allergies, and formulary status of the drug.

    + Provides computer order entry of IV’s and other medications as needed.

    + Ensures compliance with controlled substance distribution and control narcotic record keeping and documentation requirements per policy and state and federal laws.

    + Participates in patient care unit inspections as needed and assigned.

    + Label and package compounded medications and pharmaceuticals as required.

    + Responsible for compliance and knowledge of all state and federal pharmacy regulations and requirements.

    + Supervise technicians to include verification of drug order entry, filling of inpatient and outpatient prescriptions, and admixture of intravenous medications. Delegates technician functions as needed.

    + Demonstrate knowledge and competency in providing for age-specific needs of the population served.

    + Participate in community education programs to provide medication information.

    + Follows policies, procedures, and safety standards. Completes required education assignments annually. Works toward achieving goals and objectives, and participates in quality improvement initiatives as requested.

    + Performs other duties as assigned.

    Qualifications

    Minimum Education:

    None specified; however, must be sufficient to meet the standards for achievement of the below indicated license and/or certification as required by the issuing authority.

    Minimum Experience:

    None.

    Licensure Requirement:

    Current TN pharmacist license .
