ABOUT THE ROLE:
As the Lead Agency Investigator, you'll play a vital leadership role in ensuring the safety, dignity, and rights of those we support. You'll oversee investigations into allegations of abuse, neglect, or mistreatment, mentor other investigators, and ensure compliance with critical regulations. WHAT YOU'LL DO:
+ Lead with Purpose: Promote and uphold the agency's mission, Core Values, and CQL outcomes in every action you take.
+ Champion Safety & Rights: Oversee and conduct investigations into reportable incidents, ensuring protection from abuse, neglect, mistreatment, and exploitation.
+ Guide & Mentor: Provide coaching and training to Agency Investigators, reviewing reports, ensuring accuracy, and supporting their professional development.
+ Drive Excellence in Investigations: Manage the Preliminary Incident Review (PIR) committee, ensure the integrity and timeliness of investigations, and provide actionable recommendations for corrective actions.
+ Collaborate Across Teams: Serve as liaison to the NYS Justice Center, OPWDD Incident Management Unit (IMU), and other investigative bodies, while maintaining strong communication with the Corporate Compliance Officer and leadership.
+ Ensure Confidentiality & Integrity: Safeguard sensitive information, protect reporters from retaliation, and ensure documentation is complete, organized, and confidential.
+ Deliver Essential Training: Provide engaging, informative training on incident reporting and investigative procedures to employees at all levels.
+ Stay On-Call & Vigilant: Participate in the on-call rotation and conduct unannounced site visits during evenings, weekends, and overnight hours.
+ Support Compliance & Quality Assurance: Assist in developing policies, monitor regulatory updates, and support adherence to state, federal, and agency standards.
+ Use Technology Efficiently: Input and manage data in IRMA and IMS systems, ensuring timely closure of investigations.
+ Be a Team Leader: Serve on the Leadership or Supervisory Team, represent the agency on committees, and contribute to the overall quality, safety, and integrity of services.
WHAT YOU BRING:
+ Education & Experience:
+ Associate's degree with 4+ years of experience in human services, criminal justice, social work, or a related field, OR
+ NYS Licensed Registered Nurse.
+ Specialized training in Investigative Reviews preferred.
+ Knowledge & Skills:
+ Strong understanding of §624 regulations, NYS Justice Center protocols, Jonathan's Law, and the needs of individuals with intellectual and developmental disabilities.
+ Exceptional organizational skills, sound decision-making abilities, and the ability to remain objective in sensitive situations.
+ Proven ability to manage time, mentor others, and present complex topics effectively.
+ Tech-savvy with experience in basic computer applications and incident management systems.
+ Other Requirements:
+ Ability to work flexible hours, including evenings and weekends.
+ Valid NYS Driver's License.
