    Summary: The Patient Centered Medical Home (PCMH) is a model for organizing primary care practice(s) around the core functions of primary health care. Within a PCMH the care model is comprehensive, patient- centered, coordinated, and accessible with a focus on delivering high quality, cost-effective, safe care with optimized clinical outcomes and satisfaction. The Care Coordination Quality Data Analyst assists the Director in shaping the delivery of care by providing support to the implementation and ongoing management of the PCMH model of care within and across all CharterCARE Medical Associates (CCMA) Primary Care practice locations.

     

    Education: Bachelor/undergraduate preferred.

     

    Experience: At least one year of relevant work/intern experience in a

     

    medical/physician’s offices, managed care, or health system operations.

    Skills and Abilities:

    + Knowledge of basic software applications to include electronic health records.

    + Working knowledge of the AthenaHEALTH electronic medical record (EMR).

    + Knowledge of managed care principles and risk contracting.

    + Provide support to process development and improvement.

    + Ability to handle multiple priorities simultaneously.

    + Ability to organize, present and communicate clearly.

    + Ability to maintain confidentiality of patient and employee information.

     

    Working Conditions, Physical Environment and/or Safety Requirements: General pleasant working environment with little or no distractions or exposure to unpleasant or hazardous elements. Meets all safety training requirements associated with the health system. Duties are a combination of extensive mental and manual skills. Manual skills are required such as finger, eye, and hand dexterity for such activities as utilization of a computer.
