    Responsibilities:

    GENERAL PURPOSE:

    The Rehabilitation Therapist assigned to the ACT team is responsible for providing psychosocial assessment and treatment to individuals ages 18 and older who are severely and persistently mentally ill. This is provided through the clinician’s participation on an ACT outreach team designed to provide comprehensive community based psychiatric care. The Rehabilitation Therapist functions as a Vocational Specialist, per Office of Mental Health ACT Guidelines, for assigned patients and provides direct clinical services including individual, family, group interventions, crisis intervention, and community outreach. Additional services include psychosocial rehabilitation services to assist individuals manage and resolve symptoms of mental illness in school or workplace settings and maintain functional skills necessary to maintain employment or attain educational goals.

    ESSENTIAL FUNCTIONS

    + Participates as a member of an interdisciplinary treatment team providing in community treatment to assess progress toward meeting the goals of the care plan.

    + Provides feedback to the treatment team regarding client’s progress in recovery and facilitates and supervises the placement of clients in other community programs as necessary to meet recovery goals and/or needs.

    + Comprehensively documents all clinical findings, assessments and encounters in the EMR, per Hospital guidelines.

    + Knowledgeable of models of supported employment/Individual Placement and Support (IPS). Knowledge of psychosocial rehabilitative services Assertive Community Treatment (ACT) Teams Guidelines.

    + Demonstrates the ability to complete a vocational assessment to determine mental health needs in the workplace or education (e.g., career profile). Demonstrates skills development related to choosing, securing, and maintaining employment and educational goals.

    + Assists clients in maintaining themselves in the community, including setting up financial budgets, applying for entitlements, and learning skills of daily living, and other case management tasks as indicated.

    + Transports clients as needed to appointments and to meet treatment plan goals.

    + Ensures that Hospital and Department policies and procedures are implemented with regard to professional practice, patient care, and disposition training.

    + Subscribes to the Department’s goal of creating a culturally competent environment by treating patients/families, trainees, research subjects and co-workers in a sensitive and caring manner with appropriate attention to cultural differences. Participates in available cultural competence events and trainings.

    + Participates in a client on-call phone rotation with other ACT team members approximately every 3-10 weeks, as needed and according to staffing.

    QUALIFICATIONS

    + Bachelor’s Degree in an appropriate human services field and;

    + or an equivalent combination of education and experience required.

    + 2-3 years of experience providing direct services to individuals diagnosed with serious mental illness, intellectual/developmental

    + disabilities, alcoholism/substance abuse required; or experience effectively linking individuals with services that address social determinants of health required
