    Practice Location/Hours: This position will be located at Pennsylvania Hospital and will be 4x10's from 7a-5p with 1 weekend rotation requirement every 5-6 weeks. This position will offer Post call day for Mondays following a working weekend. This APP will also be required to work 1 holiday per year.

    Summary:

    We are seeking a highly motivated Inpatient and Outpatient Cardiology Advanced Practice Provider (APP) to join our dedicated cardiology team. This position offers a unique opportunity for an APP to work in both the inpatient and outpatient settings, providing high-quality care for patients with a wide range of cardiovascular conditions. This is a role that offers graded autonomy with appropriate oversight, with the opportunity to significantly impact patient care, manage acute and chronic cardiovascular diseases, and collaborate with a multidisciplinary team.

    Key Responsibilities:

    + **_Inpatient Care:_**

    + Manage a diverse caseload of consultative cardiology patients in an inpatient setting, including those in critical care units.

    + Perform comprehensive cardiovascular assessments, including physical exams, history taking, and interpretation of diagnostic tests.

    + Formulate treatment plans and order necessary diagnostic tests and medications.

    + Provide education and counseling to patients and their families regarding cardiovascular health, post-procedural care, and lifestyle modifications.

    + Collaborate closely with cardiologists, hospitalists, nursing staff, and other specialists to ensure high-quality care.

    + **_Outpatient Care:_**

    + Conduct outpatient evaluations and follow-up care for patients with cardiovascular conditions, including but not limited to heart failure, arrhythmias, hypertension, coronary artery disease, and valvular heart disease.

    + Initiate and adjust treatment regimens, including pharmacologic management and lifestyle interventions.

    + Interpret diagnostic tests, including stress tests, cardiac rhythm monitors, echocardiograms, and make recommendations for further management.

    + Build long-term relationships with patients and provide ongoing monitoring and preventive care to optimize cardiovascular health.

    + Coordinate care with multidisciplinary teams, including primary care providers, cardiologists, and other specialists.

    + **_Autonomous Practice:_**

    + Exercise independent clinical judgment in the diagnosis and management of cardiovascular patients across both inpatient and outpatient settings.

    + Provide direct care under the supervision of cardiologists, but with the freedom to make critical decisions, prescribe medications, and order tests autonomously.

    + Demonstrate a high level of competence in managing complex cardiology cases, ensuring that patient care aligns with evidence-based practices.

    + Be an advocate for the patient and work autonomously to provide personalized care, ensuring optimal outcomes and satisfaction.

    Other Responsibilities:

    + Provides direct clinical care for patient population according to established protocols and utilizes evidence based practice standards in collaboration with team.

    + Maintains accurate and confidential medical records. Documents all medical evaluations, diagnoses, procedures, treatments, outcomes, education, referrals, and consultations consistent with organizational standards in the electronic health record (EHR).

    + Identifies self properly to the patient and family, and clarifies the role of the nurse practitioner if necessary.

    + The nurse practitioner should function within their scope and refer to the collaborating physician when outside of scope, and/or directed by the collaborating physician or when requested by the patient and/or family.

    + Performs interventions specific to assigned patient population and collaborative agreement.

    + Participates in the performance improvement processes to meet quality of care and service standards.

    + Demonstrates awareness of professional responsibility as role model to peers and members of the health care team.

    + Perform approved therapeutic or diagnostic procedures based upon patient’s clinical status.

    + Addresses patient and family concerns and creates a therapeutic relationship to provide support.

    + Provide patient education, counseling and instruction regarding patient diagnosis, medical treatments, medications, and preventative care.

    + Participate in and support accreditation, compliance and regulatory activities of the organization.

    + Precept NP students and other health care learners.

    + Identifies patient/family needs for community resources and refers appropriately.

    Credentials:

    + ACLS (For providers in the acute care setting) required.

    + BLS/CPR, as a healthcare provider as per the American Heart Association (For providers in the ambulatory setting) required.

    + PA and/or NJ CRNP licensure (CRNP) required.

    + PA and/or NJ Prescriptive Authority required.

    + PA and/or NJ RN licensure (RN) required.National Board Certification.

    Education or Equivalent Experience:

    + Master of Arts or Science (Required)

    + Education Specialization:Master's Degree or Doctor of Nursing Practice (DNP) in NursingEquivalent Experience:•And New to practice or < 2.5 years, The CRNP must be reviewed by the Credentials Committee and approved by the Medical Board.

     

    All nurse practitioners and physician assistants will function under the guidelines as designed by the University of Pennsylvania Medical Center medical staff guidelines and are credentialed through that body. The CRNP or PA hired by the Department of Medicine, Division of Cardiology will be held accountable through a formal job description, through the delineation of privileges, and through their agreement with their collaborating/supervising physician who has initiated a Collaborative Practice Agreement. All clinical work hours and associated requirements will be defined and managed by the Clinical Manager for Heart and Vascular Advanced Practice, and within the human resource guidelines.
