General Summary Statement
The CRNP is responsible for applying advanced nurse practice skills for patients undergoing interventional cardiac procedures or percutaneous intervention in the Cardiac Catheterization (Cath) Lab. This includes, pre and post orders. They are also responsible for rounding and discharging post PCI patients. This could be the same day or the next day. The CRNP is also responsible for performing Stress Tests. The position requires expert communication skills, and the ability to work collaboratively with a multi-disciplinary health team. The position also requires a strong knowledge base in the field of Cardiology, with the ability to read EKGs, and manage all patients in the Cardiac population.
Essential Duties and Responsibilities
The interventional Advanced Practice Provider(APP) completes pre-cath/pre-admission workups for patients scheduled for a procedure in the Cath Lab. This will include outpatients and inpatients. This includes reviewing patient records to gather data on the patients’ current health status and a review of past medical history. The APP will perform patient history, and physical examination, order pertinent lab and diagnostic studies and evaluate the interventional patient pre and post procedure. She/he will identify normal and abnormal findings, be meticulous about documentation, and work collaboratively with the Cardiology team. This would include updating the physician, RN or other healthcare team members of any changes in patients’ orders or plan of care.
Other responsibilities include the following:
Evaluate and document patient responses to therapeutic interventions; modify the plan of care to optimize outcomes.
Initiate referrals (consultations) to physician and/or other health care providers (social services, case management, nutrition, physical therapy, cardiac rehabilitation, etc.)
Provide ongoing counseling and education to patients and families regarding the disease process, new medications, risk factors and follow up information and appointment. Answer pages appropriately to guide and facilitate accurate care. Provide face to face or telephone communication to collaborating/referring/attending physicians .
Adhere to core measures standards for patients with acute myocardial infarction (AMI) and post PCI. Implement a detailed discharge plan to facilitate optimal care transition, and prevent re-admissions.
Schedule office visits for all interventional patients before they are discharged from the hospital. Identify opportunities for and provide both formal and informal education for nursing and other health care providers.
Respond to patient emergencies in inpatient setting and the Emergency Department (ED) in the Cath Lab and/or Post Interventional Unit (PINU); work in conjunction with other rapid responders and/or Code Team members as necessary.
Practice and document in a manner that supports accreditation by Society of Chest Pain Centers (SCPC), certification by The Joint Commission (TJC) for chest pain , MI and post PCI and accreditation by The Society for Cardiovascular Angiography and Interventions for Cardiac Excellence in the Cath Lab, and the achievement of Magnet status by the American Nursing Credentialing Center (ANCC)
Education Training and Experience
Minimum 3 years clinical nursing practice in Cardiology or Critical Care. Minimum 1 years advanced practice experience.
Licensure/Certification
Licensed as a registered Nurse in the state of Pennsylvania (PA)
Licensed as an Acute Care Certified Registered Nurse Practitioner in the state of PA
Current CRNP prescriptive authority in the state of PA Current basic life support (BLS)
Current advanced cardiac life support (ACLS)
