Position Summary
Collaborates with the interdisciplinary team to coordinate the overall patient care for anyone referred to Hunterdon Health System with a known or suspected diagnosis of breast cancer, facilitating a rapid, efficient transition through the system from referral to diagnosis and treatment; Provides patient education regarding diagnosis and treatment plan; facilitates referrals to appropriate services within and outside Hunterdon Healthcare to meet the physical, psychosocial, financial and spiritual needs of patients. Active member of the breast team and NAPBC accrediation standard complaince.
Position Responsibilities
1. Facilitates and Coordinates Patient-centered care, follow patients through their initial course of care through multiple areas (medical oncology, radiation, surgery, hospital) 2. Serves as patient and family advocate, providing education and addressing barriers to care 3. Demonstrates effective resource management and interprofessional collaboration 4. Participated in and contributes expertise to activities and functions that strengthen the cancer program (including, but not limited to continuous program and quality improvement, relationship building and cancer program marketing strategies, plans, develops, and implements patient centered programs as part of disease management team, participating in community screening and community cancer awareness activities, fundraising events, etc.) 5. Facilitate and create documentation for Breast Cancer Case Conferences 6. Actively seeks and participates in educational opportunities, both formal and informal, to achieve and maintain knowledge and skill related to job responsibilities 7. Adheres to and actively participates with the American College of Surgeons National Accreditation Program of Breast Center standards (including but not limited to data collection and analysis, continuing education, protocol completion and review, survivorship, etc.)
Qualifications
High School Diploma or Equivalent required
Bachelor's Degree preferred
Completion of patient navigation training and certification within 6 months of hire
Minimum Years of Experience
3 - 4 years experience in healthcare is required
Breast oncology experience preferred
Patient navigation training and certification preferred
Excellent organization and communication skills (written and verbal); Self-directed, strong team player, collaborative, calm under stress, resourceful. Commitment to continuous quality improvement and life-long learning.
