    The Social Services Specialist responsibilities include:

    + Being a member of a collaborative practice model that includes patients, nurses, hospital care coordinators, physicians, and other practitioners, caregivers, and the community

    + Provides coordination, referral and interdisciplinary discharge planning

    + They are knowledgeable about treatment, social and economic implications for individuals and families as it pertains to their age, stage of development and discharge needs

    + Possesses a clear understanding of discharge and transition planning and linkage with community resources

    + Demonstrates skills in planning, organizing, and managing multiple functions

    Position details:

    + **Location:** St Joseph's Hospital - Tampa, FL

    + **Status:** PRN (as needed), hourly

    + **Schedule:** 8:00 AM - 4:30 PM, days vary

    + **Weekend Requirement:** Occasional

    + **On Call:** No

    Certifications and Licensures:

    + Preferred ACM (Case Management) or

    + Preferred CCM (Case Manager)

    Education:

    + Required Bachelors in Social Work, Sociology, or Related Field

    + Preferred Masters Degree in Social Work, Sociology or Related Field (If MSW level, will be hired as Social Services Specialist II)

    Experience:

    + Required 6 months Internship or

    + Required 6 months Social Work in an acute care setting
