**Location:** Grand Haven | **Schedule:** Part-Time (20 hrs/week), Day Shift | One Weekend/Month
As a **Registered Nurse Case Manager** , you’ll be a vital part of our interdisciplinary team, helping patients transition safely and comfortably to the next level of care. This is a meaningful role where your clinical expertise and empathy will directly impact patients and their families during critical moments.

+ Current RN licensure in the State of Michigan
+ Four years of recent, relevant clinical experience preferred
+ Strong communication and care coordination skills

+ Conduct utilization reviews and ensure compliance with care standards
+ Meet with patients and families to support safe discharge planning
