Your Impact as a Home Health Physical Therapist:

+ Provide **one-on-one care** in clients' homes, ensuring they receive top-quality therapy.

+ Perform **mobility and functional assessments** , develop **individualized care plans** , and help clients regain strength and independence.

+ Educate clients and their families on exercises, safety measures, and strategies to enhance daily living.

+ Collaborate with a multidisciplinary team to deliver **holistic, patient-centered care** .

+ Document progress and care plans efficiently using **state-of-the-art EMR technology** .

What We’re Looking For:

+ **Active NC Physical Therapist (PT) license**

+ Graduation from an **accredited PT program** (APTA or CAHEA/AMA-approved)

+ Strong **time management, communication, and problem-solving skills**

+ Ability to work **independently** while staying connected to a supportive team
