Job Summary and Qualifications
Performs and provides professional case management services in the Rehabilitation Center in order to (1) provide for the overall coordination in the delivery of services and treatment planning, (2) ensure and document effective cost management to maximize financial resources and utilize available resources, (3) promote the development of positive relationships between the facility staff, the patient and family, funding representatives and other involved or interested parties, (4) ensure continuity of care is maintained post-discharge, (5) assist in facility compliance with all relevant standards of care and treatment, and (6) assist in the development and implementation of new products and services, staff education and training, and good management practices.
What you will do in this role:
+ You will provide program orientation to patients/families/caregivers. Orientation will include the case manager's role, Rehab philosophy, and continued stay and discharge criteria. Orientation will also include Medicare and insurance benefits, grievance procedures, treatment plan process, and rights and responsibilities.
+ You will act as the coordinator of patient/family/caregiver education. You will promote the participation of the patient/family/caregiver in team discussions related to plans, goals, and status. This will be conducted through Family Conferences and other interactions.
+ You will ensure the implementation of the patient's treatment plan that supports the patient's strengths, abilities, needs, and preferences. You will facilitate the involvement of the patient throughout the rehabilitation process.
+ You will document the findings of the Discharge Planning Evaluation (DPE) and psychosocial assessments. You will communicate the social, financial, or discharge needs and preferences of the patient/family/caregiver.
+ You will assume accountability for promoting consistent, positive patient interactions that advance the agenda of unparalleled patient service.
What qualifications you will need:
EXPERIENCE REQUIRED
+ 1 – 3 years:
+ Other required experience: The facility may elect to substitute additional years of work experience for education at its discretion. A minimum of 1 year clinical experience in a physical rehabilitation setting/ organization or 2 years experience in another related health care setting or any combination of the two.
EDUCATION REQUIRED
+ Associate’s Degree-Major: Nursing
*BSN Required if hired after 5/1/2020
Other required experience: The facility may elect to substitute additional years of work experience for education at its discretion. A minimum of 1 year clinical experience in a physical rehabilitation setting/ organization or 2 years experience in another related health care setting or any combination of the two. LICENSES AND CERTIFICATIONS
Required:
+ Licensed Registered Nurse or Clinical Social worker in the State of Texas
Required Upon Hire (for certifications only: upon hire is defined as within 2 business days of start date)
