FlexStaff is seeking a Care Delivery Manager RN for our client, a non-profit healthcare organization providing home and community-based healthcare and services for the elderly.
JOB RESPONSIBILITIES:
+ Oversee personalized care plan development and execution.
+ Facilitate IDT, SDR, and other meetings, including attestations and Virtual IDT collaboration.
+ Serve as manager on call, handling follow-ups, documentation, and grievances.
+ Run clinical delivery reports.
+ Represent CHN in IDT meetings.
+ Manage site staffing, resources, and team assignments.
+ Communicate with IDT to coordinate care based on participant needs.
+ Review UAS assessments for accuracy.
+ Investigate and address incidents, ensuring timely Level 2 investigations.
+ Ensure complete and accurate participant documentation.
+ Review charts for quality and address deficiencies.
+ Collaborate with Transitions in Care team.
+ Coordinate and arrange long-term care services (nursing homes, rehab, etc.).
+ Monitor care management effectiveness, utilization, and adjust plans as needed.
+ Ensure documentation complies with CLHC standards and regulations.
+ Collaborate with HR on interviews, coaching, disciplinary actions, onboarding, and training. Ensure regulatory compliance.
+ Evaluate direct reports' performance, provide feedback, and conduct reviews.
+ Ensure direct reports follow regulations and organizational policies.
+ Investigate and resolve employee grievances.
+ Serve as DPS for LHCSA, as assigned.
+ Act as an IDT member.
QUALIFICATIONS:
+ RN/BSN from an NLN accredited nursing school or 5+ years of healthcare experience.
+ NYSED-approved infection control coursework (8+ contact hours).
+ Case Management Certification recommended within two years.
+ 2+ years of management experience in CHHA, LHCSA, acute care, medical-surgical, critical care, nursing home, or clinic setting (preferred).
+ Customer service experience required.
+ Long-term care insurance experience a plus.
+ 1+ year of experience with frail/elderly populations (or training provided upon hire).
+ Supervisory experience required.
+ Current, active, unrestricted NY State RN license.
+ Medically cleared for communicable diseases with up-to-date immunizations.

The salary range and/or hourly rate listed is a good faith determination of potential base compensation that may be offered to a successful applicant for this position at the time of this job advertisement and may be modified in the future. When determining a team member's base salary and/or rate, several factors may be considered as applicable (e.g., location, specialty, service line, years of relevant experience, education, credentials, negotiated contracts, budget and internal equity).
The salary range for this position is $120,000-$125,000/year
