Position Summary:
The Pharmacist provides support in all pharmacy operations in a Durable Medical Equipment (DME) setting. Responsibilities include verifying and processing prescription orders, ensuring regulatory compliance, and maintaining accurate documentation. The Pharmacist works collaboratively with the care team to ensure consistent service and adherence to company protocols and industry standards.
Key Responsibilities:
+ Review, receive, and verify all new patient orders and accompanying documentation.
+ Collaborate with physicians and healthcare professionals to ensure the safe and effective use of medications.
+ Provide clinical consultations and respond to inquiries from patients, caregivers, and healthcare providers regarding the appropriate use of medications.
+ Counsel patients and caregivers on specific drug therapies, including administration guidelines, potential adverse effects, and drug interactions.
+ Monitor and assess ongoing medication therapy, including:
+ Therapeutic appropriateness of prescribed medications
+ Therapeutic duplication within the patient's regimen
+ Proper dosage, frequency, and route of administration
+ Medication adherence, potential drug/food/test interactions, and disease-specific limitations
+ Maintain thorough and accurate documentation of all clinical activities and communications within the patient record.
+ Manage the ordering, inventory, and maintenance of medication supplies.
+ Prepare, label, package, and dispense medications in accordance with organizational procedures and regulatory standards.
Qualifications & Responsibilities:
Required:
+ Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy
+ Current, active pharmacist license in good standing in [Insert State]
+ Strong clinical knowledge and understanding of pharmacotherapy principles
+ Excellent communication, documentation, and organizational skills
+ Proficiency in pharmacy systems and electronic medical records
Preferred:
+ Previous experience in home healthcare or specialty pharmacy
+ Familiarity with inventory management systems
