Are you passionate about making a difference in patients' lives? St. Luke's is seeking a dedicated Ambulatory Care Pharmacist to join our vibrant team at the Hailey Family Medicine Clinic. What You Can Expect

+ **Empower Patients:** Work under a broad collaborative practice agreement to provide expert chronic disease state medication management.

+ **Fast-Paced Environment:** Manage a dynamic clinic schedule with 8-12 patient appointments each day, ensuring efficient and effective care.

+ **Support & Educate:** Be the go-to resource for providers and patients, answering drug information questions, troubleshooting pharmacy issues, and educating on medication use.

+ **Collaborate for Success:** Coordinate with a multidisciplinary team, including physicians, advanced practice practitioners, behavioral health specialists, and care managers to deliver holistic care.

+ **Teamwork:** Work alongside clinical pharmacy technicians to enhance patient outcomes.

Qualifications

+ Degree from an accredited pharmacy program

+ Valid Idaho Pharmacist License

Preferred Qualifications

+ Previous experience with the clinical management of primary care based chronic diseases

+ Completion of PGY2 Ambulatory Care ORcompletion of a PGY1 pharmacy residency program with ambulatory care-focus ORequivalent ambulatory care experience

+ Current Board Certification in Pharmacotherapy (BCPS), Ambulatory Care Pharmacy (BCACP), or Geriatric Pharmacy (BCGP)
