    Responsibilities

    Serve as a professional resource to patients, physicians, and patient families as well as community organizations/providers and schools to address issues and concerns relating to patient care services.

    Responsibilities:

    + Establishe contact and rapport with patients and their families, and provide periodic updates to family members.

    + Intermediate between the patient, family members, physicians, community organizations, schools and care providers, as needed to facilitate communication and resolve issues.

    + Establishe follow up services, ensuring that all items on the patient care plan have been completed; collaborate with the next level of care including arranging for appointments as indicated.

    + Address customer service issues with proper follow-up or referral, use service recovery resources, and communicate issues and resolution to appropriate level.

    Requirements

    Licensure / Certification / Registration:

    + BLS Provider preferred. American Heart Association or American Red Cross accepted.

    Education:

    + High School diploma equivalency with 2 years of cumulative experience OR Associate'sdegree/Bachelor's degree OR 4 years of applicable cumulative job specific experience required.
