Responsibilities
Provide behavioral health counseling services.
+ Conduct patient assessments and establishe therapy treatment goals.
+ Administer individual and group therapy.
+ Maintain thorough and confidential records of observations.
+ Evaluate progress and effectiveness of therapy interventions.
+ Provide patient with information regarding resources and therapeutic activities in the community.
+ Work directly with the patient, family, physician and community providers to ensure active and collaborate care from the initiation of treatment to the time of discharge.
Requirements
Licensure / Certification / Registration:
+ One or more of the following required:
+ Clinical Counselor credentialed from the Illinois Department of Financial and ProfessionalRegulation obtained prior to hire date or job transfer date.
+ Therapist specializing in Marriage & Family credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.
+ Licensed Clinical Social Worker (LCSW) credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or transfer date.
+ Licensed Clinical Professional Counselor (LCPC) credentialed from the Illinois Department ofFinancial and Professional Regulation obtained prior to hire date or job transfer date.
+ Social Worker specializing in Clinical Practice credentialed from the Illinois Department ofFinancial and Professional Regulation obtained prior to hire date or job transfer date.
+ BLS Provider preferred. American Heart Association or American Red Cross accepted.
Education:
+ High School diploma equivalency with 2 years of cumulative experience OR Associate'sdegree/Technical degree OR 4 years of applicable cumulative job specific experience required.

Maternal Medicine experienced STRONGLY PREFERRED
Required Licensure: LPC or LSW
Preferred Licensure: LCPC or LCSW
