Description
Prevention Counselor
Northern Rivers Family Services
As a Prevention Counselor, you will provide supportive counseling and case planning services to county residents. Your focus will be to help parents, caregivers and children succeed by drawing on their natural strengths while addressing the risks that impact safety, permanency and well-being. You will provide services in the home, school and community. You will report to Clinical Coordinator of the Parsons Schenectady Prevention Program. You will work a flexible schedule with some remote work opportunities.
Responsibilities:
+ Maintain a caseload of up to ten families and meet with each family weekly.
+ Engage and sustain engagement of families throughout course of treatment.
+ Assess individual(s) and family(s) including use of assessment tools to the position.
+ Identify and discuss the strengths, risk factors and service needs which influence the health and safety of a client/family.
+ Develop with the family and other service providers mutually agreed upon treatment goals.
+ Facilitate Family Team Meetings and other service plan reviews to assess progress and identify any needed modifications to treatment planning.
+ Provide therapeutic and case planning services to children and families.
+ Establish and maintain working relationships with colleagues, case managers and collaterals.
+ Participate in weekly or bi-weekly supervision and program Team Meetings.
+ Maintain case record and complete all paperwork: status sheets, progress notes, service plans, correspondence, discharge summaries. Observing program contract, agency policy, COA and state mandates.
Requirements and Education:
+ Bachelor’s degree in Social Work or related field with three to five years of human services experience required.
+ Valid driver’s license and reliable access to a vehicle required
+ General knowledge of the child welfare system and an ability to work with children and adults required.
