    Description

    Behavior Intervention Specialist, Clinical Time Transition Program

     

    Work related travel reimbursed at predetermined rates

     

    As a Clinical Time Transition Program (CTTP) Behavior Intervention Specialist, you will provide assessment, intervention, and support for youth aged 11-17 transitioning from a hospital setting back to their community. You will work with families, schools, and community resources to promote successful reintegration and long-term stability. You will report to the Director of the Hospital to Home (H2H) Program, and you will work onsite with community travel.

    Responsibilities:

    + Conduct functional behavior assessments (FBAs) and develop individualized behavior intervention plans (BIPs).

    + Assess strengths, challenges, and need related to the transition process.

    + Collaborate with hospital teams to develop discharge and transition plans.

    + Implement evidence-based behavioral interventions tailored to individual needs.

    + Train caregivers, school staff, and community providers in behavior management strategies.

    + Support skill-building in areas such as emotional regulation and social interactions.

    + Work with families to provide education and support.

    + Coordinate with schools to ensure continuity of care and appropriate accommodations.

    + Connect youth with community-based services and resources to foster long-term stability.

    + Track progress using data-driven methods and adjust interventions.

    + Conduct regular check-ins with youth, families, and service providers.

    + Maintain detailed records and documentation in compliance with ethical and regulatory standards.

    + Provide training to caregivers, educators, and community staff on behavioral strategies.

    + Offer crisis management support and consultation.

    Requirements and Education:

    + Requiresoneof the following options:

    + National Board certification in behavior analysis (BCBA) and a Master's degree in behavior analysis or a related field.

    + Master's degree in a clinical or treatment field of psychology, social work, or related human service field, or a New York State license in Mental Health Counseling, and specialized training in functional assessment techniques and behavior support plan development.

    + Bachelor's degree in a human service field, plus documented experience in the OPWDD system since December 31, 2012, and active pursuit of a Master's degree

    + Knowledge of behavioral principles, trauma-informed care, and transition planning required.

    + 1+ years of experience working within a clinical, hospital, or community-based setting.

    + 1+ years of experience with developmental disability needs and systems.

    + Must pass a physical.

    + Valid NYS Driver’s License required.
