The OP Care Coordinator - Housing Specialist will be responsible for lease renewals and collaboration between both tenant and landlord as it pertains to their housing documentation/leasing needs. This role will also be responsible for assisting with the completion of intakes for new program referrals

Responsibilities:

Assessment & Documentation

+ Conducts assessments to identify consumer needs and documents findings accurately.

+ Maintains case records, intake paperwork, and tracks data using DSM and internal systems.

Case Management

+ Links consumers to services and coordinates care across providers.

+ Ensures continuity of care through collaboration with internal teams and community supports.

Consumer Support

+ Promotes recovery and self-management through education and advocacy.

+ Guides consumers in navigating agency and community resources.

Team Collaboration

+ Works with treatment teams to manage caseloads and address engagement barriers.

+ Participates in meetings and consults on clinical and administrative matters.

Reporting & Compliance

+ Completes documentation, referrals, and reports in a timely and accurate manner.

+ Bachelor’s Degree in human services or Business related fields.

Licensure, Certifications, and Clearances:

+ Comprehensive Crisis Management (CCMC)

+ Driver's License

+ Act 34 with renewal

+ OAPSA with renewal
