    We are actively looking for a Case Manager BSW to join our team!

     

    + Job Title: Case Manager, Bachelor's in Social Work (BSW)

    + Schedule: PRN, 8:30 a.m. - 5:00 p.m.

    + Must be able to work 3 days a week during training.

    + Department: Case Management

    + Location: Thomasville Medical Center

     

    The BSW Case Manager serves to facilitate complex discharge planning and is a valued member of the healthcare team. They collaborate with physicians and other members of the healthcare team to assess, plan, implement and evaluate proactive, complex and safe discharge plans. Collaborates with RN/MSW Case Manager for guidance with medically and/or psychosocially complex cases. The BSW Case Manager along with the other members of the healthcare team accepts responsibility, authority and accountability for management of care. This shall be in accordance with the current policies and procedures, mission, vision and values of Novant Health. Serves as a resource for discharge planning and care coordination.     Qualifications

    + Education: 4 Year / Bachelors Degree, required. Bachelor's Degree in Social Work from school accredited by the Council on Social Work Education, required.

    + Experience: Two or more years professional social work experience or two or more years healthcare experience, preferred. Case Management or related experience, preferred.

    + Licensure/Certification: NC state approved curricula for restrictive interventions (facility specific) within 3 months of hire for Emergency Departments and Behavioral Health units, required.

    + Additional Skills (required): Weekend and holiday coverage.

    + Additional Skills (preferred): Knowledge/Competence of Microsoft office products and keyboarding
