    Description:

    Consultation/teaching with diabetic patients: 30- 60 minutes which included in depth diabetic teaching, including carbohydrate counting & plate method, insulin administrations. Treatment of high & low glucose.

     

    * Nurse visits scheduled for diabetes follow ups when MD appt were too far out.

    * Initiate the process for CGM placement including downloading app, connecting to clinic, troubleshooting.

    * Apply continuous glucose monitors (CGM’s), review CGM & insulin pump downloads.

    * Discuss various insulin pumps on market and it’s use. Placing initial pump orders & arranging for pump training.

    * Working with patients and their insurance company to order CGM’s & glucometers.

    *Teach patients how to check blood glucose.

    *Initiate prior authorizations for medications

    Qualifications:

    Must be a Certified Diabetes Care and Education Specialist (CDCES)
