    Description

    Discharge Planning Coordinator / RN

     

    Oncology

     

    Shift: Full Time 7:00 am - 5:30 pm

     

    + Contributes to excellence in patient/family centered care by coordinating the health care of select patient populations/designated services across the continuum of care using the nursing process.

    + Promotes the safety and well being of all patients by coordinating the patient's discharge from the hospital and transition of care to the out-patient clinic.

    + Collaborates with the inpatient interdisciplinary team to develop and maintain the plan of care.

    + Works in collaboration with others to ensure that all discharge needs are met. Schedules all discharge appointments, treatments, and protocol specific orders.

    + Completes needed discharge follow up phone calls to patients and documents appropriately.

    + Performs other duties as required.

    + Works with manager to formulate plan for professional development.

    + Attends educational in-services as appropriate.

    + Participates in professional activities and organizations to maintain knowledge of current trends, practices, and developments.

    Minimum Qualifications:

    + Bachelors degree in Nursing.

    + Must have a valid, active unencumbered Nursing license or temporary permit approved by the Georgia Licensing Board.

    + Three years of clinical nursing experience as a Registered Nurse with at least two years of recent experience (within the last five years) in the area of concentration.

    + OCN certification preferred for some positions.

    + BLS required; ACLS Certification preferred but may be required in some areas.
