Summary of Qualifications:
The ideal candidate will be self-motivated and possess strong organizational skills to meet both immediate and long-term deadlines. Maintaining strict confidentiality while managing a significant workload is essential. Effective verbal and written communication skills, along with strong interpersonal abilities, are required. We highly value teamwork in our environment, and professionalism paired with problem-solving skills is a must.
Primary Duties and Responsibilities:
As a social worker at Forest Hills, your professional judgment will be crucial in supporting the well-being and quality of life of our residents. Your responsibilities will include:
+ Attending interdisciplinary team meetings to ensure quality care.
+ Meting government standards for social service documentation.
+ Keeping accurate records of residents’ psycho-social needs and their progress.
+ Collaborating with team members and Administration to uphold residents’ rights and mental wellness.
+ Staying informed about community charges, legislation, and programs affecting nursing home residents.
+ Working closely with residents’ families, and staff to provide support and resources, encouraging residents to advocate. For their needs.
+ Completing social histories and psycho-social assessments to understand each residents’ social, emotional, and psychological needs.
+ Assisting in the development of written care plans that address residents’ psycho-social needs and outline goals, and interventions.
+ Providing support to help residents adjust to life in a long-term care facility, with a focus on their emotional and psychological well-being.
+ Coordinating discharge planning and referring residents to appropriate hoe care services before their transition back to the community.
+ Possess a current, unencumbered District of Columbia LICSW license.
+ Experience in counseling, including work with clinical assessments.
+ Knowledge of nursing and medical practices and procedures (preferred).
+ Strong leadership and supervisory skills with the ability to organize, plan, develop, implement, and interpret policies and procedures.
