Facility:Dayton Children's - Tech TownDepartment:PFK Care CoordinationSchedule:Full timeHours:40Job Details:The Care Coordinator Social Worker ensures quality patient care in the appropriate setting by assessing key needs, addressing local and systemic issues interfering with care, assisting families in setting appropriate goals and tracking progress toward achieving those goals. Navigates the health care system on behalf of high need patients (including delegates); supervises and provides care coordination; plans for education; and provides for patient safety and quality of care. Serves as an agile problem solver on multiple levels, from patient-level, to disease population and care system. Practices professional social work and promotes patient advocacy in accordance with the NASW Code of Ethics for Social Work.
Department Specific Job Details:
+ full time, 40 hours
+ day shift, M - F
+ 8am - 4:30pm
+ no weekends or holidays
+ serving the mental health population
Education Requirements:
Masters: Social Work (Required)
Certification/License Requirements:
LSW Ohio - Ohio Licensed Social Worker - Counselor, Social Worker and Marriage & Family Therapist Board
