    Position Overview:

    This leadership role is responsible for developing and overseeing population health programs and value-based care initiatives to achieve high-quality health outcomes, expand access to care, and reduce costs. The role requires a strategic, data-driven approach and close collaboration with Clinical, Network, Operations, and Health System leaders as well as external stakeholders to drive innovation, expand capabilities, and integrate population health strategies into overall value-based care governance and performance. The ideal candidate is a change leader, innovator, and action-oriented collaborator who excels in leading multi-disciplinary teams and delivering results.

     

    **Job Description** :

    Education** **:

    + Bachelor’s degree in healthcare administration, public health, business, or a related field required.

    + Master’s degree in public health (MPH), business administration (MBA), or a related field preferred.

    Experience** **:

    + Proven track record of strategic leadership in population health programs and value-based care initiatives.

    + Experience in leading multi-disciplinary teams and successfully launching and managing care management programs.

    + Significant experience collaborating with clinical, operational, and external stakeholders to drive innovation and expand healthcare capabilities.

    + Demonstrated ability to integrate population health strategies into value-based care governance and performance structures.

    + Minimum of 8-10 years of experience in a similar role or within the healthcare industry.

    Knowledge** **:

    + Strong understanding of population health management principles, value-based care models, and healthcare systems.

    + Knowledge of data analytics and insights for driving strategic healthcare initiatives.

    + Familiarity with healthcare regulations, policies, and quality improvement programs.

    + Awareness of current trends and innovations in population health and value-based care.

    + Strong collaboration and relationship-building skills across multidisciplinary teams and external stakeholders.

    Pay Range is $204.33 to $276.44 / hour

     

    _The salary range for this role may vary above or below the posted range as determined by location. This range has not been adjusted for any specific geographic differential applicable by area where the position may be filled. Compensation takes into account several factors including but not limited to a candidate’s experience, education, skills, licensure and certifications, department equity, training and organizational needs.
