    **HARBORVIEW MEDICAL CENTER SOCIAL WORK** has an outstanding opportunity available for a **PEER NAVIGATOR (SOCIAL WORK ASSISTANT) to join the MADISON CLINIC** .

    WORK SCHEDULE

    • Full-Time / 40 hours per week

    • Day Shift

    DEPARTMENT DESCRIPTION

    • The Madison Clinic is an interdisciplinary, outpatient primary care clinic, located within Harborview Medical Center, which serves individuals living with HIV as well as those who are at high-risk of acquiring HIV.

    • The Madison Clinic case management program receives grant funding to provide financial advocacy to patients who are receiving HIV care at Madison Clinic. Many of the patients served at Madison Clinic have complex psychosocial needs and financial advocacy is a critical component of their healthcare access.

    • Many of the patients served at Madison Clinic have complex psychosocial needs that make navigating the health care system challenging, therefore peer navigation is vital for supporting patient’s health literacy, treatment adherence and engagement in medical care.

    POSITION HIGHLIGHTS

    • The purpose of being a Peer Navigator is to serve as a role model for living well with HIV

    • Provide guidance to people living with HIV (PLWH) helping patients overcome barriers to engagement, adherence, and retention in treatment. Some examples of the barriers experienced by Madison Clinic patients include lack of adequate insurance, substance use, stigma, complicated immigration statuses, mental health issues, and homelessness

    • Work in a multidisciplinary team setting, alongside providers, case managers, nurses and financial advocates to facilitate the patient’s linkage to medical and psychosocial care

    • Focus services on patients who are newly diagnosed, justice involved, transferring care from clinics/organizations outside Harborview and patients needing to reengage in care

    PRIMARY JOB RESPONSIBILITIES

    • Help patients navigate the HIV/medical care system and work in collaboration with case managers to assist patients with accessing needed services.

    • Provide one-on-one and/or group social and emotional support.

    • Work directly with patients to support medication adherence and treatment engagement.

    • Work in collaboration with the patient’s care team including medical case managers, doctors, and/or other staff involved in the patient’s care to provide coordinated support for their engagement in care.

    • Address HIV treatment adherence needs and coach patients to improve adherence skills. Follow-up with patients who have missed appointments and identify barriers to engagement in HIV care.

    • Build rapport with patients to facilitate engagement and retention in medical care, adherence and other supportive services to address well-being, self-sufficiency and reducing barriers to care.

    • Where feasible, accompany patients to appointments, provide reminders, so they can effectively access HIV care, SUD treatment, mental health care and other supportive services.

    • Adhere to HMC/Social Work Department standards for documentation and confidentiality, complete documentation in compliance with grant deliverables.

    REQUIRED POSITION QUALIFICATIONS

    Bachelor of Social Work or a Bachelor’s Degree in the behavioral sciences AND two years experience as a Social Work Assistant I or equivalent.

    OR

    an Associate of Arts Degree in the behavioral sciences AND four years of work experience as a Social Work Assistant I or equivalent.
