    The **PerformCare** -PA Behavioral Health Clinical Care Manager (BH-CCM) is a core participant of the collaborative treatment team, including the member/family, behavioral health providers, county behavioral health service system, Physical Health (PH) - Managed Care Organization (MCO) and medical Providers when applicable. The BH-CCM approves care coordination, utilization management, and some types of case management per assigned caseload. Behavioral Health referral management, PH-BH, referrals, follow-up, authorization determinations, connection to natural/community supports including SDoH supports, and quality of care are key components of the BH CCM role.

    Education/Experience:

    + **Master's Degree** ;in Social Work, Mental Health Counseling, Psychology, and/or Nursing; **required**

    + **Pennsylvania** License - LCSW, LSW, LPC, LMFT, or RN is; **required**

    + Registered Nurse is required to have a Master's degree and two (2) years of experience in Psychiatric Adult direct practice Behavioral Health/Psychiatric direct services

    + Minimum **three (3) years** of postgraduate clinical experience or equivalent combination of education and clinically related work experience

    + Minimum **one (1) year** of direct practice experience in Adult Behavioral Health Service Delivery System; **required**

    + Knowledge base in Medicaid, Medicare, and/or Commercial Managed Care Systems **;required**

    + Behavioral Health Utilization Management experience and/or experience in Mental Health Inpatient and/or Mental Health Partial Hospitalization **preferred**

    + An understanding of the Pennsylvania treatment services and community services is; **preferred**

    + An additional **Substance Abuse** certification is **preferred**
