In conjunction with the admission nurse and Directors of Homecare and Inpatient Services, responsible for the assessment of referrals for hospice services for appropriateness. Reports to the Administrator.
Job Duties and Standards:
+ Process every hospice referral on same day as referral, unless otherwise indicated.
+ Process referrals by obtaining sufficient admission and insurance information in order to ensure appropriate and timely admission of patients. Scans patient records into EMR and upload to patient charts. Orders SRK, and coordinates equipment delivery on approved admissions when needed.
+ Provide patient records for hospice eligibility admission, utilizing approved criteria and guidelines. Provides billing with information to verify insurance information and eligibility.
+ Provide information regarding available community resources and other community resources when hospice care is not appropriate.
+ Provide timely and consistent feedback to referral sources, physicians, and patient/families.
+ Communicate accurately and completely to physicians, staff members, patients, families, and supervisors.
+ Maintain communication with pending patients, their families, and referral sources and routinely follow the progress of prospective patients and those referrals not taken under care.
+ Participate as a member of the marketing team by developing and maintaining positive relationships with referral sources; participate in staff meetings, department meetings, staff meetings, in-services, committees, community-education events and other activities as needed.
+ Prepare daily Huddle Report and Bi-Weekly Appropriate Care report regarding referral statistics and pending referrals. Ensure high level of integrity of referral data within Company computer system.
+ Participate in orienting new staff, students, volunteers and board members.
+ Participate on committees and projects, as assigned.
+ Act as a resource to peers, students, orientees and other staff members.
+ Participate in education process of the Company, both to the medical and lay communities as appropriate.
+ Serve as telephone triage for incoming phone calls from patients and/or families
+ Ensure timely attention to customer concerns and complaints.
+ Provide information about hospice services to all requesting it.
+ Provide outstanding customer service.
+ Assist in collecting feedback from referral sources.
+ Participate in assigned hospital/facility committee(s) as requested.
+ Work cooperatively with all departments to meet the needs of the community and patient/family.
+ Maintain regular contact with hospital and cancer centers’ discharge planners and/or social workers and other key hospital staff. Contacts patient/family to initiate admission.
+ Accurate data entry into company computer system.
+ Demonstrate knowledge of the different levels of hospice care and work with clinical staff for appropriate utilization of Morrison Manor and continuous care as part of the hospice continuum of care.
+ Coordinates the completion of the Certification of Terminal Illness from providers. Completes CTI Narrative for Medical Director using medical records. Enters appropriate diagnosis codes on all patient using medical director CTI/Recert.
+ Order Management:
+ communicates with attending physicians, hospice staff members and other agencies as needed to coordinate optimal care and use resources fo rthe patient/family.
+ Keeps admission packets made and available to staff.

+ Minimum Requirements: High school diploma, health care experience
