SOCIAL WORKER - Full Time
Work with residents by identifying their psycho-social, mental and emotional needs along with providing, developing, and/or aiding in the access of services. Foster a climate of compassion with the interdisciplinary team.
Duties and Responsibilities include the following. Other duties may be assigned.
Administrative:
+ Review policies and procedures to assure compliance with state and federal regulations; Participate in reviewing and recommending policies concerning resident care and quality of life.
+ Develop, maintain and utilize a list of community resources that are useful to residents and their families.
+ Understand and meet all government requirements for social work documentation, e.g. MDS, Admission, discharge and updating care plans.
+ Documents progress in meeting the psychosocial needs of the resident daily in the EMR.
Clinical:
+ Complete social history utilizing electronic medical record and psychosocial assessment for each resident that identifies social, emotional and psychological needs.
+ Participate in the development of an interdisciplinary plan of care for each resident that identifies the psychosocial needs and issues of the resident, the goals to be accomplished for those needs, and the appropriate social worker interventions.
+ Provide therapeutic interventions to assist residents in coping with their transition and adjustment to a long-term care facility, including social, emotional and psychological needs.
+ Provide support and education to resident families to assist in their understanding of placement and facility issues in addition to referring them to the appropriate agencies/services when the facility does not provide the needed service.
+ Provide clinical interventions to address significant events that occur during a resident stay.
+ Coordinate and plan resident discharges and make referrals for appropriate home care services prior to the resident's return to the community.
Advocacy:
+ Work with the interdisciplinary team and administration to promote and protect resident rights and the psychological wellbeing of each resident. Prevent and address resident abuse as mandated by law.
+ Identify community changes and opportunities such as legislation, regulations and programs that affect nursing home residents within the area of social work.
+ Work with residents, families and staff to provide support, information and organization and support to improve the quality of care for individual residents.
Education:
+ Educate residents and families regarding their rights and responsibilities, effective problem solving and the extent of community, health and social services that are available to them, including those necessary for effective discharge planning.
General:
+ Work in close collaboration with physicians, nursing, dietary, Rehab (OT, PT, Speech) and other health care personnel in patient evaluation and treatment to further their understanding of significant social and emotional factors underlying patient's health problem.
+ Help residents and family through individual or group care plan meetings to understand, accept, and follow medical recommendations.
+ Provide service planned to restore patient to optimum social and health adjustment within patient's capacity.
+ Utilize community resources to assist resident to resume life in community or to learn to live within limits of disability.
+ Prepare resident histories, service plans, and reports.
+ Participate in planning for improving health services by interpreting social factors pertinent to development of program.
+ Provide general direction and supervision to workers engaged in clinic home service program activities.
Education, Training, Experience
+ Bachelor's degree (BA/BS.) in Social Work or related field required. Masters in Social Work and LMSW preferred.
+ Previous experience in long-term care setting is required.
+ Must have outstanding communications and interpersonal skills and be able to successfully navigate the responsibilities as a patient advocate, family advocate and clinical team member.
+ Knowledge of Point Click Care.
