    For 80 years, we have been one of upstate New York’s largest providers of programs and services for individuals who have disabilities. Many of the innovative programs and vital services that we offer are not available elsewhere.

     

    We are searching for an LPN-Float Health Care Coordinator to join our agency. This position contributes to the Mission of the Center, the Nursing Department, and Residential Program by providing and overseeing the medical management for the people we support assigned to his/her caseload. The work schedule for this position is Sunday-Thursday 12PM-8PM. Responsibilities:

     

    + Provides direct oversight for health and medical issues of the people we support

    + Maintains regulatory compliance of required documents

    + Provides oversight for direct care staff, regarding topics related to and associated with maintenance of the health and safety of the people we support

    Requirements:

    + NYS LPN License required

    + High School Diploma or equivalent

    + At least 2 years of related experience required, 2 years of related experience working in the field of Intellectual and Developmental Disabilities preferred

     

    Pay range for this position is $24.48 - $26.52. Pay is commensurate on experience.
