    Northwest Florida Community Hospital (NFCH), in Chipley, FL, is seeking a Full Time experienced, energetic, strong and dedicated Licensed Practical Nurse II (LPN II) to provide exceptional care for patients in support with the medical Providers, during the day shift, in our Med Surg Unit.

    The successful candidate will be responsible for the following duties:

    • Work closely with other Nurses and Nursing Assistants to divide care tasks for patients.

    • Provides direct patient care in an organized and timely manner. Demonstrates necessary skills in providing age appropriate care and documents per unit policy.

    • Initiates and assists with emergency measures as directed by RN in charge.

    • Understands of all equipment on acute care units, including but not limited to; beds, scales, IV pumps, enteral feeding pumps, SCDs, respiratory equipment, etc.

    • Maintains current knowledge of medications and their correct administration based on age specific criteria, patient clinical condition and safety policy.

    • Transcribes physician orders for medications to the MAR correctly; charting correct medication, dose, route, frequency and timing all doses to be given correctly.

    • Compiles a complete list of the patient’s medications to assist in reconciliation of medications across the continuum of care.

    • Maintains and updates patient charts per unit policy. Checks each chart for completeness prior to releasing record to health information.

    • Provides appropriate discharge instructions to patients and family members in a timely manner. Includes instructions and referrals based on patient/family needs and physician orders.

    • Must demonstrate dependable, regular attendance

    • Must be able to communicate and function productively on an interdisciplinary team

    • Provides a safe environment for patients and family members. Reduce the risk of patient falls by keeping patient rooms and areas free of excess clutter.

    • Complies with infection control standards.

    • Ensuring all confidentiality and privacy rights of residents are observe.

    • Supports and promotes the Mission, Vision, and Values; adheres to all hospital policies and procedures; commits to the Standards of Conduct; maintains professional appearance.

    • Other duties as assigned.

    The successful candidate will possess the following minimum qualifications:

    • Licensed as a Practical Nurse in the State of Florida and in good standing with the Board of Nursing

    • Excellent interpersonal skills, including customer service, patience, empathy and compassion

    • Effective communication skills, including active listening, writing, speaking and reading comprehension

    • Good collaboration and teamwork skills

    • Fast and adaptive problem-solving abilities

    • Keen attention to detail

    • In-depth understanding of patient care policies and regulations

    • Physical strength and stamina

    • Excellent computer and communication skills.

    • Follow all established Federal, State and Local rules, regulations and guidelines.

    Physical Requirements:

    Must be able to sit, stand, bend, lift, push, pull, stoop, walk, reach, and move intermittently during working hours

     

    Must be able to lift 50 lbs. floor to waist, lift 50 lbs. waist to shoulder, lift and carry 50 lbs., and push/pull 50 lbs.
