Job Summary
Working under general direction, coordinates and provides social services for patients of all ages, applying principles of a highly professional field. Services are provided in accordance with the philosophy, mission, standards, policies, and procedures as ordered by the physician.
Essential Duties and Responsibilities
Professional Responsibilities:
+ Provides a positive and professional representation of the organization.
+ Promotes culture of safety for patients and employees through proper identification, reporting, documentation, and prevention.
+ Maintains hospital standards for a clean and quiet patient environment to maintain a positive patient care experience.
+ Maintains competency and knowledge of current standards of practice, trends, and developments in related scope of job role or practice.
+ Adheres to infection-control policies and protocols, medication administration and storage procedures, and controlled substance regulations.
+ Participates in ongoing quality improvement activities.
+ Maintains compliance with organization’s policies, as well as established practices, protocols, and procedures of the position, department, and applicable professional standards.
+ Complies with organizational and regulatory policies for handling confidential patient information.
+ Demonstrates excellent customer service through his/her attitude and actions, consistent with the standards contained in the Vision, Mission, and Values of the organization.
+ Adheres to professional standards, hospital policies and procedures, federal, state, and local requirements, and standards from accrediting bodies.
Core Duties:
+ Develops and maintains social service program, drafting supporting policies, procedures, and guidelines for the program.
+ Communicates with physicians to report patient progress, needs, concerns, and potential and actual problems.
+ Works closely with all members of the health care team in development of the plan of treatment, including discharge plans, based on the psychosocial assessment and accepted social work interventions.
+ Develops and maintains thorough knowledge of referral resources throughout the service area.
+ Develops working relationship with leaders in community resource groups, representing agency at meetings and on various committees that address social needs of patients, such as Council on Aging, Developmental Services, etc.
+ Acts as resource person for other members of the health care team, providing in-service education and consultation in understanding the social and emotional factors related to health problems.
+ Maintains up-to-date knowledge of Medicare, Medicaid, and other third party reimbursement sources.
+ Maintains accurate and timely records, including clinical and progress notes, according to department policy.
+ Participates in planning and implementing quality improvement activities.
+ Maintains up-to-date knowledge of field by participating in continuing education activities.
+ Provides positive customer relations in dealing with patients, families, physicians, fellow department employees, other hospital departments, referral resources, service agencies, and others.
+ Adheres to all department/hospital policies.
+ Supervises social work students as approved by the director and administrator.
+ Ability to provide quality social work services in the form of discharge planning, crisis intervention, financial information and resources, high risk intervention and reporting to appropriate agencies.
+ This position provides services to both inpatient and outpatient units at SCRMC.
Qualifications Education and Experience
+ Current LPN license by the Mississippi Board of Nursing
+ Must be able to read, write, understand verbal and written communication
Preferred Qualifications
+ CPR Certification
Minimum Requirements Essential Skill Competencies:
+ Body system assessment
+ Phlebotomy: Establish IV access (LPN II only)
+ Medication monitoring
+ Preventive skin care
+ Aseptic wound care/ dressing change
+ Foley catheter insertion/care
