    Pay Grade: $64.07-$84.89*

     

    A Pharmacist is responsible for ensuring the safe and effective use of medications within the healthcare setting. The pharmacy department services patients of all ages from neonatal to geriatric. Pharmacists collaborate with all professional staff and ancillary services to maximize safe and quality patient care. Some primary duties include providing drug information services, medication profile reviews, medication therapy monitoring, renal dosing, drug dosing as well as sterile compounding. The pharmacist ensures that patient care is maintained through compliance with the department's policies, rules and regulations promulgated by the Board of Pharmacy, and other applicable regulatory bodies such as CMS and The Joint Commission.

    Summary of Job Responsibilities: (Not all inclusive)

    + Interdisciplinary care teams:

    + The Pharmacist may participate in interdisciplinary care rounds or other committee meetings such as medication safety, antibiotic stewardship, Pharmacy and Therapeutics, etc.

    + Works collaboratively with the interdisciplinary team to maximize individual patient outcomes.

    + Performs review of patient’s home medications at admission.

    + Compares medication history against inpatient orders to ensure accuracy.

    + Works with physicians on discrepancies found between inpatient orders and medication reconciliation and documents outcomes.

    + Medication therapy monitoring.

    + Ensures drug regimens are appropriate and follows hospital established policies/procedures/protocols or evidence-based guidelines in the absence of hospital policies/procedures/protocols. This includes but is not limited to compliance with antimicrobial use guidelines, IV to PO interchanges, renal dosing, and serum drug concentration protocols.

    + Review of medication orders.

    + The Pharmacist evaluates medication therapy, identifying medication related problems. All medication orders are reviewed for the appropriate dose, duration, frequency, dosage form and indication.

    + Monitors therapy for potential or actual drug interactions and contraindications.

    + Monitors selection of safe, appropriate, cost-effective medication therapies according to established formulary, policies and procedures.

    + Provides drug information to health care providers.

    + Answers general and patient specific drug information questions.

    + Participate in direct patient teaching and discharge counseling.

    + Sterile compounding.

    + Compound sterile intravenous compounds while adhering to 797 regulations.

    + Other duties as assigned. Pharmacists supervise the work of technicians, may be involved in policy and procedure upkeep and development, controlled substance diversion monitoring, ambulance restocks, order set review and development. May be asked to participate in education programs as needed.

    Qualifications:

    Education:

    + Doctor of Pharmacy (PharmD) degree or BS in Pharmacy from an accredited pharmacy school.

    Licensure:

    + Valid state license to practice as a pharmacist in New York.

    Experience:

    + ( Highly preferred. Not required but candidates with this experience will be prioritized): 1 to 2 years hospital experience.

    + (preferred, not required): 2 years of leadership experience.

Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
