    SUMMARY:

    The DCF Navigator works with and within SERV Connect’s Crisis Stabilization and Assessment Programs and must be person-centered and team oriented. This individual will work closely with upper management, group home managers, CSOC, Care Managers from many different organizations, families of the youth we serve, and legal counsel (at times). The DCF Navigator must always remain professional, have exceptional communication skills, and work efficiently and effectively under time constraints. This person will facilitate all admissions and discharges in the program. This person will also act as the communication contact for all members of the Child Family Team for the youth that we serve in our programs. Ensure that services are rendered according to SERV Connect’s mission and SERV BHS’ ethical standards. The DCF Navigator will cooperate with the licensee, Office of Licensing and Division of Children and Families staff in any inspection or investigation. Performs additional duties as deemed necessary by supervisor.

    EDUCATION AND EXPERIENCE:

    + Must be 21 years of age.

    + Bachelors level practitioner with 3-5 years of relevant experience with individuals with Intellectual and Developmental Disabled (I/DD) individuals or an unlicensed master’s level practitioner with 1 year of the same experience.

    + A driver's license, in good standing, issued in the current state of residence.

    SKILLS:

    + Ability to work closely with other team members of all levels Required. Ability to work a flexible schedule, including occasional evening or weekend hours, when deemed necessary.

    + Experience with "Cyber" preferred.

    + Experience with "Perform Care" and CSOC Required.

    + Highly organized and Team Oriented Required.

    + Ability to read, write and communicate well.

    + Ability to accurately and thoroughly document in "DCF CSOC CYBER" platform and the individual’s charts and treatment plan.

    + Ability to do simple math.

    + Basic computer skills and knowledge of Microsoft Word and Excel.

    + Knowledge of current principles and practices of behavioral healthcare, specific to the developmentally disabled population.

    + Ability to bend and reach during an eight hour day.

    + Able to lift a minimum of 10 pounds and drive a company van for one to two hours a shift.

    + Visual and auditory acuity sufficient to evaluate information and maintain accurate records.

    + Fine motor skills for legible and accurate writing of reports, daily correspondence and presentation either manually or orally.

    WORK CONTEXT:

    The employee will be working with individuals who have intellectual and/or developmental disabilities.  Individuals served may also have a behavior intervention plan.  The employee may be required to transport individuals served as part of their routine employment with SERV.

    PRIMARY JOB DUTIES:

    + Conduct a family orientation in the first 24 hours

    + Review and sign all required paperwork and consent within the first 48 hours of admission.

    + Demonstrate collaborative relationships with system partners, including CMO and DCP&P (if involved), and knowledge of the system of care procedures and resources.

    + Daily check-in with program staff to obtain necessary information for transition planning.

    + Contact clinicians and group home managers frequently to discuss the status of their youth’s transition plan

    + Contact with parent/caregiver at minimum twice per week in order (more frequently as deemed necessary) to discuss status of their youth’s transition plan

    + Provide, as needed, on-site family psycho educational activities tied to comprehensive treatment and discharge plan monthly;

    + On a weekly basis, will gather input from all team members and enter a weekly summary in the youth’s progress notes within CSOC’s CYBER.

    + Work closely and provide program reports with Quality Team

    + Assist with the implementation of Quality surveys to families and youth.

    + Facilitate and Attend all Admissions and Discharges.

    + Regular contact with the Director of Behavioral Support Services and to provide updates.

    + Other responsibilities as needed/directed.

    + Submit monthly reports to CSOC regarding critical position changes and vacancies.

    SALARY:

    + $66,951.56 per year.
