The Opioid Stewardship Clinical Pharmacist will provide pharmacotherapeutic support to the entire enterprise including inpatient and outpatient settings. As such the role is expected to provide clinical leadership in best practices for use of opioids and treatment/prevention of opioid use disorders.
This pharmacist will participate in multidisciplinary committees and will also serve as a preceptor in the Atlantic Health System PGY1 Pharmacy Residency Program.
Additional responsibilities include:
+ Review patient medication regimens and providing recommendations to optimize therapy based on clinical guidelines and best practices.
+ Conducting medication reconciliation to ensure accuracy and safety during transitions of care.
+ Educating patients and caregivers on proper medication use, side effects, and adherence strategies.
+ Collaborating with healthcare providers to develop and implement patient-specific care plans.
+ Monitoring patient outcomes and adjusting therapy as necessary to achieve therapeutic goals.
+ Providing drug information to healthcare professionals and patients as needed.
+ Contributing to quality improvement initiatives related to medication use and patient safety.
+ Documenting interventions, clinical assessments, and recommendations in the electronic health record.
+ Staying current with developments in pharmacotherapy and evidence-based medicine.
+ Serve as a preceptor and mentor for pharmacists, pharmacy residents and students as needed
+ Participate in direct patient care rounding and activities in any setting as needed
+ Augment the education of healthcare and non-healthcare team members on medication use; serve as a preceptor and mentor for pharmacists, pharmacy residents and students.
+ Participate in site-based and system-wide multi-disciplinary committees.
+ Conduct medication use evaluations and research projects in collaboration with other healthcare professionals to identify areas of medication use optimization with the goal of publication and/or presentation at local and/or national meetings.
+ Responsible for comprehensive medication management to patient populations across the enterprise with focus on pain management, palliative care, and substance use disorder
+ Identify patients at higher risk for adverse outcomes, monitor safe opioid prescribing, dispensing, administration and appropriately manage pain outcomes
+ Provide periodic updates to AHS stakeholders on quality initiatives and outcomes
+ Educate team members on best practices in opioid stewardship to support safe and effective opioid use and management
Education/Experience
+ Must be a graduate of an accredited school of Pharmacy with PharmD degree required.
+ Completion of a PGY1 residency preferred. Board certification in pharmacotherapy (BCPS) preferred.
+ Current NJ Board of Pharmacy license required.
+ Minimum of 3 years’ experience in a hospital practice setting preferred.
+ Experience in research activities also preferred.
