General Summary
With matrix reporting to hospital leadership, supervises and oversees the work activities of Case Management at specified entity. Works in a team leadership model to develop and maintain departmental operations that are consistent with Hospital, System and community needs. Provides direction to staff regarding case management, discharge planning, social services and utilization management. Provides input into the budget management of resources and works collaboratively with Case Management and Hospital leadership to oversee the around-the-clock operations of case management.
Duties and Responsibilities
+ Oversees the hiring of case management staff. Trains/orients personnel, evaluates performance, prepares work schedules and makes recommendations for personnel actions.
+ Participates in developing budgets.
+ Identifies needs in program planning and facilitates program development. Supervises patient care, family communication, and record keeping.
+ Collaborates with other hospital programs and community agencies with which patients and families are involved.
+ Collaborates with hospital leadership to support operational goals and the Case Management support of those goals and participates/ represents case management with hospital committees.
+ Prepares and maintains written documentation as required by the profession and the department.
+ Actively monitors standard work flows and provides guidance to staff on issues involving discharge planning, payer relations and utilization review.
+ Monitors and shares department Key Performance Indicators and informs hospital and system leadership of areas for improvement and works with leadership and staff to address/ resolve.
+ Engages with staff to develop lean daily management process to access opportunities for improvement and empowers team to address. Conducts employee/ leadership rounding with staff to identify and resolve barriers for staff.
Common Expectations:
+ Demonstrates knowledge of case management, utilization review and population health. Assesses and interprets patient age specific data and provides appropriate, age specific treatment.
+ Demonstrates a commitment to patient, visitor and staff by: complying with all applicable safety regulations; learning the impact of medical errors and methodology that will lead to reduction of errors; reporting actual and potential errors, as well as hazardous conditions; identifying opportunities to standardize processes and "error proof" systems that will lead to increased safety; and participating in safety education programs and root cause
+ Maintains established policies and procedures, objectives, quality assessment, safety, environmental and infection control standards.
+ Enhances professional growth and development through participation in educational programs, current literature, inservice meetings, and workshops.
+ Provides outstanding service to all customers; fosters teamwork; and practices fiscal responsibility through improvement and innovation
+ Acts as liaison with hospital operations nursing/medical staff and community agencies to facilitate problem solving and coordinate services. Acts as an educational resource.
Qualifications
Minimum Education:
+ Bachelors Degree Required
+ Masters Degree Preferred
Work Experience:
+ 3 years Supervisory experience. Required
+ Lean Daily Management/quality improvement experience. Preferred
Licenses:
+ Licensed Registered Nurse Upon Hire Required or
+ Registered Nurse Multi State License Upon Hire Required
Courses and Training:
+ Clearance per Child Protective Services law (11 P.S. 2201) amended by PA Act 33 of 1985. Upon Hire Required
