Description:
Pharmacist - Northern Network (Whitehall / Shelby-Hart / Ludington)
+ Outpatient clinics and physician offices.
+ Anticoagulation clinic setting.
+ Occasional travel between clinic sites may be required.
Clinical Practice:
+ Provide comprehensive medication management for chronic disease states such as diabetes, hypertension, hyperlipidemia, asthma/COPD, osteoporosis and heart failure.
+ Conduct patient assessments, develop care plans, and monitor therapeutic outcomes in collaboration with the healthcare team.
+ Manage anticoagulation therapy (e.g., warfarin, DOACs) including patient education, INR monitoring, dose adjustments, and documentation.
+ Participate in transitions of care to reduce hospital readmissions and improve continuity of care.
+ Complete comprehensive medication reviews as required.
Collaboration & Integration:
+ Serve as a key member of the care team within physician practices.
+ Provide drug information, therapeutic recommendations, and support to providers and staff.
+ Assist in the development and implementation of clinical protocols and pathways.
+ Work under a collaborative practice agreements.
+ Education & Training:
+ Educate patients and caregivers on medication use, adherence, and lifestyle modifications.
+ Precept pharmacy students and residents as applicable.
+ Provide continuing education and training to healthcare professionals.
Quality Improvement & Documentation:
+ Document clinical interventions and patient encounters in the electronic health record (EHR).
+ Participate in quality improvement initiatives and data collection to demonstrate clinical and economic outcomes.
+ Ensure compliance with regulatory and accreditation standards.
Minimum Qualifications:
+ Doctor of Pharmacy (PharmD) degree from an ACPE-accredited institution.
+ Completion of a PGY1 Pharmacy Practice Residency (PGY2 in ambulatory care preferred).
+ Current pharmacist license in Michigan.
+ Certification in Anticoagulation Management or equivalent experience.
+ Strong clinical knowledge and communication skills.
+ Proficiency with EHR systems and clinical documentation.
Preferred Qualifications:
+ Board Certification in Ambulatory Care Pharmacy (BCACP) or Pharmacotherapy (BCPS).
+ Experience working with and utilizing a collaborative practice agreement setting.
