    SUMMARY:

    The Clinical Pharmacist provides distributive and clinical pharmacy services in a professional and competent manner to assure rational and appropriate drug therapy for all patients.

    RESPONSIBILITIES:

    + Pharmaceutical Management: Provide specialized pharmaceutical services to improve drug usage and therapeutic outcomes; advise providers on issues concerning drug therapy, side effects and dose management.

    + Leadership & Relationship Development: Mentor/supervise pharmacists in training; coordinate the activities of assigned pharmacy technicians; maintain liaison relationship with medical and nursing staff.

    + Evaluation: Conduct and evaluate medication histories, assess compliance and suggest modifications.

    REQUIRED QUALIFICATIONS:

    + Degree in Pharmacy from an accredited college of Pharmacy.

    + Licensure as a Pharmacist in New York State.

    PREFERRED QUALIFICATIONS:

    + Minimum 4 years of pharmacy practice experience in a hospital setting.

    + Strong analytical, computer, and communication skills with an excellent attention to detail.

    BS: Pharmacy (Required)

    LICENSES / CERTIFICATIONS:

    LPharm - Licensed Pharmacist - New York State Education Department (NYSED)

    PHYSICAL REQUIREMENTS:

    M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.

    $120,000.00 - $165,000.00

    The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
