    Job Description

    The person will conduct investigations when allegations of abuse/ neglect or significant incidents have occurred in our SPDD and PCR programs. In addition to work with all of the SPDD residential and day habilitation staff to collect data and use evidence to develop measurable goals for program improvement using a Continuous Quality Improvement approach. This person will also ensure programs’ compliance w/agency policies and government regulations from NYS OPWDD, JC, DOH, and OMIG. This staff person will report to the Director and Assistant Director of QI and Program Development.

We operate programs in the New York Metropolitan area and Hudson Valley.

    Key Responsibilities

    + Assists with Incident tracking, monitoring and trend analysis.

    + Conducts investigations for all reportable and serious reportable incidents as assigned.

    + Ensures that all written investigation reports are completed and prepared for IRC review and upload to IRMA within the required 30 day timeframe.

    + Uploaded written 149 investigation reports and all relevant supporting documents into the NYS Justice Centers WSIR system.

    + Ensures that all incident related documentation including 149 investigation reports meets acceptable criteria for submission to the Incident Review Committee.

    + Conduct on-site case reviews.

    + Assess data from a variety of methods and share findings with program staff.

    + Participate in collaborative problem solving with program staff to address immediate issues and strengthen program operations.

    + Ensure that agency is in compliance with all Federal, OPWDD, OMIG, OMH, DOH and local regulations.

    + Identify areas of concern and work with Program Leadership to create systems to address these concerns.

    + Assist with management and tracking of information in OPWDDs IRMA database and interface with the Justice Centers WISR and AARM to ensure incident closure.

    + Maintains records and database of investigations completed and submit information as requested for preparation of the Annual Trend Analysis and other tracking reports.

    + Any other related duties, as required.

     

    About You

     

    You are the ideal candidate if you are interested in furthering your administrative skills while making an impact on your community. Being organized, well dispositioned, detail oriented and responsive are critical attributes to be successful in this role.

     

    + Proficiency in core competencies is important, including: data collection, assessment, collaborative problem solving, facilitation, written communication, strategic planning and investigations.

    + Bachelor's Degree Required with at least 2 years of experience

    + Ability to problem solve, and work independently

    + Candidate must possess a current Investigator Certificate from the Justice Center, OPWDD or OMH.

    + Experience/knowledge working with OPWDD and NYS Justice Center.

    + Available on-call via agency phone on a 24 hour/7 days per week basis and a flexible schedule for staff interviews.

    Job Type: Full-time

     

    Pay: $55,000.00 - $60,000.00 per year
