Job Title:Family Support Navigator

Job Summary:
TheFamily Support Navigator (FSN)plays a vital role in FLACRA’s mission by providingpeer support servicesto family members of individuals experiencing seriousmental illnessand/orsubstance use disorders. Working under thesupervision of the Program Manager, the FSN will serve as amentor, advocate, and support systemfor families, helping them navigate the challenges of addiction recovery, mental health treatment, and available resources.
This position requiresflexible working hours, including evenings and weekends, to meet theunique needs of familiesin the community. FLACRA employs aholistic, integrated approachto family support, ensuring access toeducation, peer networks, treatment options, and community servicesto help loved ones in recovery.
Required Qualifications:
+ Completion ofNY State-certified peer support specialist trainingthrough OMH or OASAS.
+ Peer Support Specialist Certification (CRPA-P or CRPA)required.
+ Valid NYS Driver’s Licenserequired.
+ CRPA-F certificationstrongly encouraged within the first year of hire.
+ Tier 1:Participation in first available peer certification training, acquiringCRPA-P no later than four months after employment.
+ Tier 2:Certification as aCRPA-P or CRPA.
Skills & Competencies:
+ High School Diploma or GEDrequired.
+ Must be18 years or older.
+ Excellent interpersonal, written, and verbal communication skills.
+ Strongproblem-solving, time management, and crisis intervention skills.
+ Knowledge ofpeer support models, addiction recovery principles, and behavioral health services.
+ Experience working with diverse populationsand the ability to engageculturally diverse individuals and families.
+ Proficiency in electronic record-keepingand familiarity withcase documentation.
+ Comfort with providing community-based services, includinghome visits and outreach efforts.
+ Ability to workindependently and as part of a multidisciplinary team.
+ Valid NYS Driver’s Licenserequired; ability to travel within the Finger Lakes region.
+ Availability forovernight on-call coverage as part of a rotation.

Key Responsibilities:
Client & Family Support:
+ Engagewith families, caregivers, and individuals to provideemotional support and advocacy.
+ Assist clients indeveloping and implementing recovery plans, includingcrisis plans.
+ Providecase management, referrals, and service coordinationto connect families withtreatment options, peer networks, and support groups.
+ Support families in understandingaddiction, opioid use disorder, and recovery processes.
+ Offereducational guidanceon treatment programs, harm reduction, and wellness strategies.
Community Engagement & Collaboration:
+ Cultivaterelationships with local addiction prevention, treatment, and recovery organizations.
+ Work withmanaged care organizations, community coalitions, and service providersto help families access necessary treatment and services.
+ Conductoutreach effortsto increase awareness of family support services.
+ Advocate forfamily-centered policiesand improved access to resources.
Teamwork & Leadership:
+ Serve as anactive team member, participating intreatment planning meetings and team case discussions.
+ Providebackup supportto COTI team members during crises or urgent situations.
+ Maintaindetailed, accurate case documentationin compliance withOASAS and agency regulations.
+ Participate inmonthly staff meetings, in-service training, and professional development opportunities.
+ Tier 2 and Tier 3 peersare expected to serve asleaders and mentors, offering guidance and support toTier 1 peers.
Crisis Intervention & On-Call Support:
+ Provide crisis interventionservices to individuals and families struggling with addiction and mental health challenges.
+ Participate in aweekly overnight on-call rotationto offersupport to clients in crisis.
+ Ensure timelydocumentation and reportingof all crisis interventions.
